Sinusitis is an inflammation, or swelling, of the tissue lining your sinuses. Sinuses are structures inside your face that are normally filled with air. Bacterial infections, viral infections and allergies can irritate them, causing them to get blocked and filled with fluid. This can cause pressure and pain in your face, nasal congestion (a stuffy nose) and other symptoms.

Causes:

1)Viral Infections: The most common cause of sinusitis is a viral infection, often from the common cold or flu. These infections can cause the sinuses to become inflamed and block the normal flow of mucus.

2)Bacterial Infections: Sinusitis can also be caused by bacterial infections. When the sinuses are blocked, bacteria may grow and cause an infection. This type of sinusitis is often more severe and may require antibiotic treatment.

3)Allergies: Allergic reactions to pollen, dust, mold, or other allergens can trigger sinusitis. The body’s immune response to these allergens can lead to inflammation of the sinuses.

4)Nasal Polyps: Growth of small, non-cancerous outgrowths in the nasal passages, known as nasal polyps, can obstruct the sinuses and contribute to sinusitis.

Symptoms:

  • Nasal Congestion: A common symptom of sinusitis is nasal congestion or a blocked nose. This occurs due to the inflammation and swelling of the sinuses, hindering the normal flow of air.
  • Facial Pain and Pressure: Individuals with sinusitis often experience pain and pressure in the face, particularly around the forehead, eyes, and cheeks.
  • Headache: Sinus headaches are another common symptom. The pain is often more pronounced in the forehead and may worsen with movement or bending over.
  • Postnasal Drip: Excess mucus production can lead to postnasal drip, causing a sore throat and cough. This can be especially bothersome at night.
  • Reduced Sense of Smell and Taste: Sinusitis can affect the olfactory system, leading to a diminished sense of smell and taste.

Types of sinusitis : 

According to the duration, sinusitis can be classified as:

1 )Acute sinusitis – which runs less than four weeks, which is most common

2)Sub acute Sinusitis – which runs from 4 to 8 weeks

3)Chronic Sinusitis – which persists longer than 8 weeks

Locations of sinus cavity and associated symptoms:

1)Maxillary sinus – pain or pressure in maxillary areas (tooth, cheek, headache, etc.)

2)Frontal Sinus – pain or pressure in the frontal sinus areas (above eyes, frontal headache)

3)Ethmoid Sinus – pain or pressure between/ behind eyes

4)Sphenoid sinus – pain or pressure behind the eyes, but often refers to the vertex .

What Causes  Sinusitis According To  Ayurveda?

According to Ayurveda, the main site of kapha dosha is the head, and the nose is the gateway to the head. Hence, similar symptoms of sinusitis are mentioned under Nasa roga (disease of the nose) and Shiro roga (disease of head). It can be correlated as Kaphaja siroroga and Suryaavartha under Siroroga and also as Dushtapratishyaya in Nasa roga.

Nidana (Causative Factors)

Ahara Nidana

o Atibhojana (excessive intake of food)

o Guru bhojana (excessive intake of food taking a long time to digest)

o Ati snigda bhojana (excessive intake of oily food)

o Atyambupaana/sheethambupaana (excessive drinking of cold water)

Vihara Nidana

o Aasya sukham (sedentary lifestyle)

o Svapna sukham (excessive sleep/daytime sleeping)

o Rajo, dhooma, sheetha vayu sevana (exposure to dust, smoke, cold breeze)

o Ritu vaishamya (sudden climate change)

o Vegodharana (suppression of urges, especially chardi (vomiting) or tears)

o Use of too high or too low pillow

  1. Mantra recitation – chanting a mantra that encourages air to move back and forth between the sinus membrane and nasal passages. Air movement helps open the tiny ducts, or ostia that connect the nose to the sinuses, allowing the sinuses to drain correctly. This can help prevent infections in the sinuses and create a healthy environment therein.
  2. Jala neti (nasal irrigation with lukewarm saline water) – Helps in the expulsion of dried mucous from the nose, cleanses sinuses and helps to create an unobstructed flow of air.
  3. Sutra neti (nasal cleaning with thread) – It improves mucociliary clearance, thinning the mucus and decreasing the inflammation that

blocks the sinus ostia.

  1. Gomukhasana (cow face pose) –Stretches the chest muscles, aiding flexibility in the air passage region and reducing stress and anxiety.
  2. Janu Sirsasana (Head to knee pose) – Relieves headaches, fatigue, anxiety, and insomnia.
  3. Setu Bandhasana (Bridge pose) – Helps reduce depression and anxiety.
  4. Bhastrika Pranayama –This Pranayam frees the lungs of unnecessary air and microbes and facilitates drainage of the excess mucous from the sinuses and nasal mucosa.
  5. Anulom Vilom Pranayama/NadisodhanaPranayama-Removes congestion from the Nadis, thus ensuring free flow of energy throughout the body, boosts the immune system and helps to overcome the imbalance of autonomous nervous system.
  6. Bhujangasana (cobra pose) –Makes breathing easier by giving stretch to the lungs and works as a ] stress-releasing mechanism.
  7. Kapalabhati (frontal brain cleansing exercise) –The finest exercise for cleansing respiratory blockages, improves mental function, refreshes and rejuvenates the head region.

Stress can worsen the symptoms of sinusitis. Mental stress reduces microcirculation to the sinuses; whenever stress is reduced by meditation, microcirculation is improved. This improves our immunity and prevents frequent bouts of sickness.

Yoga and Pranayama:

Engaging in specific yoga asanas and pranayama (breathing exercises) can improve respiratory health and support sinus drainage. Practices such as Bhujangasana (Cobra Pose) and Anulom Vilom (Alternate Nostril Breathing) are beneficial.

Maintain Regular Sleep Patterns:

Adequate and restful sleep is crucial for overall well-being and immune function. Establishing regular sleep patterns aligns with Ayurvedic principles and supports the body’s natural healing mechanisms.

Stay Hydrated:

Drinking warm water throughout the day helps maintain hydration and supports the digestive process. Proper hydration is essential for reducing inflammation and promoting optimal bodily functions.

Apathya (don’ts)

  • Cold food items like ice-cream, juices, etc.
  • Sour and salty food
  • Dairy products
  • Heavy, deep fried, processed or junk food
  • Excess sweets/sugar
  • Refined flour
  • Day or excessive sleep
  • Cold water shower
  • Frequent head bath
  • Exposure to cold climate, air-condition
  • Cigarette smoking
  • Swimming in a chlorinated pool

Sinusitis  Ayurvedic Treatment

The principle line of treatment is ama pachana (to break down toxins) by giving medicines with deepana pachana (digestive) to increase Agni (digestive fire). Then, to clear the obstruction of the srothas (channels), teekshana Shodana (substantial expulsion of aggravated)  to the nearest route, i.e. nose by Nasya (one of the panchakarma) is given. Further, to enhance the immunity of the individual, Rasayana is advised. In all types of siroroga and nasaroga, nasya is the prime treatment to remove the aggravated doshas. If the patient has more bala (strength) and aggravated doshas, Vamana is also recommended.

There are two modalities of treatment

  1. Shodana (Purificatory treatment)

 a)Deepanapachana (to increase Agni)

 b)Sirovirechana

  1. Shamana (Palliative treatment)

 a)ayurvedic medicines and Herbal Formulations

The treatment mainly  includes Deepana pachana, Snehana, Swedana, Vamana, Virechana, Dhoomapana, and Nasya. 

Deepana-Pachana

The importance of Deepana-Pachana drugs is to bring the Sāma Dosha to Nirāma state. Thus, Deepana-Pachana should be administered in the beginning of the Snehana therapy depending upon the conditions, when it arises. Deepana Pachana drugs are used to increase the Agni and for the digestion of Ama.

 Snehana/Mukhabhyanga

 Abhyanga causes Mriduta of Doshas and according to modern science, massage to a specified area causes increased blood circulation.

 Swedana Karma

 Swedana causes Vilayana (liquefaction) of accumulated Doshas (mucous). Vamana Karma 

Charaka defined Vamana as a process in which morbid Dosha are eliminated through upper channels i.e., mouth. Chakrapani mentions Urdhavabhaga as Urdhavamukha. Bhavaprakasha also has same opinion for Urdhva as Mukhamarga. Vamana is a process in which Apakva Pitta and Kapha are removed forcefully through upper channels.

Virechana Karma

Virechana (therapeutic purgation) is a common procedure that is widely practiced among the Panchakarma treatments (pentad treatments). Virechana  increase the secretion by irritating the mucous membrane of the GIT. Second, it prevents absorption of nutrients, and finally, it increases gastrointestinal motility. 

Dhoomapana Karma 

Dhumapana works on Vata and Kapha Dosha. Due to Sukshma Guna of drugs used for Dhumapana; it opens/enters the smallest channels, with Ushna and Tikshna Guna it liquefies and eliminates the Dosha from their nearer routes. However gaseous form of medicine increases the bio availability of it. The gases are absorbed in blood by pressure difference and greater surface area in lung. By using the particular process of Dhumapana i.e., uses three puffs of smoke without exhalation, one can achieve its absorption at maximum level.

Nasya Karma/Shirovirechana

Nasya therapy is a crucial component in Ayurvedic treatment for sinusitis. This therapy involves the administration of herbal oils or medicated substances through the nasal passages. Nasya helps lubricate the nasal passages, clear congestion, and reduce inflammation, effectively relieving nasal symptoms.
